

Non-Administrator Coordinated Home Care Program means a Coordinated Home Care Program which does not have an agreement with the Claim Administrator or a Blue Cross Plan but has been certified as a home health agency in accordance with the guidelines established by Medicare.Ĭomputer database or “database” means a collection of recorded information in a form capable of, and for the purpose of, being stored in, processed, and operated on by a computer. Small employer carrier means any carrier that offers health benefit plans covering eligible employees of one or more small employers in this state.Įquity Account Administrator means the brokerage firm utilized by the Company from time to time to create and administer accounts for participants in the Company’s equity plans and programs, including the Plan.Įducation Record means those records that contain information directly related to a student and which are maintained by an education agency or institution or by a party acting for the agency or institution. Program Administrator means the person retained by the Wisconsin PACE Commission as provided in subsection (5)(b).Īdministrative Committee means the committee in charge of Plan administration, as described in Article VII. Securities Depositories means DTC and, in accordance with then current guidelines of the Securities and Exchange Commission, such other addresses and/or such other securities depositories as the District may designate in a Written Request of the District delivered to the Paying Agent. Program participant means that term as defined in section 3 of the address confidentiality program act.

Plan Administrator means the plan administrator described in Article 8.Įducation records means an education record as defined in the Family Educational Rights and Privacy Act and its implementing regulations, 20 USC Section 1232g and 34 CFR Part 99, respectively. Participant’s records means the records prepared and maintained by the Contractor (in whatever form or storage media) concerning an individual Participant.

Participant Direction Procedures means such instructions, guidelines or policies, the terms of which are incorporated herein, as shall be established pursuant to Section 4.10 and observed by the Administrator and applied and provided to Participants who have Participant Directed Accounts.ĭepository System Participant means any participant in the Depository’s book-entry system. Recordkeeper means a “financial intermediary” as defined in Rule 22c-2 of the Investment Company Act.
